- ab aap box me applicant ka Name , Father’s Name , Date of Birth , Address , Previous Name fill kre

- officer ka full name , officer Designation(पद का नाम) fill kre , officer ke Department ka name fill kre , officer Identity card ka number fill kre , or mobile fill kre
