Print the pdf first
After that affix the photo as given below
Sign and take care that the right side does not go outside the signature box. After that fill the Pan number. Now select the title and fill the name of the applicant.

उसके बाद निचे दिए गए चित्र के अनुसार फोटो चिपकाए
और signature करे ध्यान रखे की right side signature बॉक्स के बहार न जाये और उसके बाद Pan number fill करे
अब title select करे और आवेदक का नाम fill करे
Male : Shri
Woman : Smt
Girl : Kumari
Company & Firm : M/s
If the name of the applicant is “ MANISH ” then MANISH will be written in Last Name / Surname And First Name, Middle Name will remain empty. Here the applicant’s name is “MANISH YADAV”, then we will write “YADAV” in Last Name / Surname and “MANISH” in First Name and Middle Name will be left blank.

यदि आवेदक का नाम “MANISH” है तो MANISH को Last Name / Surname में लिखा जायेगा और First Name ,Middle Name खली रहा जायेगा
यहाँ पर आवेदक का नाम “MANISH YADAV” है तो हमें “YADAV” को Last Name / Surname मे और “MANISH” First Name में लिखा जायेगा और Middle Name खली रहा जायेगा

♦ Step #2
Now fill father’s name here and select “Father’s name” below, if you are filling mother’s name then select “Mother’s name”.
अब यहाँ पर पिता का नाम fill करे और निचे “Father’s name” select करे यदि आप माँ का नाम fill करे रहे हो तो “Mother’s name” select करे
♦ Step #3
select Gender and fill the date of birth. And you fill the type of address “RESIDENCE” means that you are filling the same address which is on your aadhar card, then you will have to select “RESIDENCE” if you want any other
Must select “OFFICE” Note :- If you select “OFFICE” then you have to upload “Annexure-B” https://mypancenter.com/annexure-b/ form along with document
Gender select करे और जन्म तिथि भरे
और आपने address का टाइप भरे “RESIDENCE” का अर्थ होता है की जो आपके आधार क्रेड पर address है आप वो ही address fill कर रहे हो तो आपको “RESIDENCE” select करना होगा यदि आप और कोइ
“OFFICE” select करना होगा नोट :- यदि आप “OFFICE” select करते हो तो आपको “Annexure-B” https://mypancenter.com/annexure-b/ फॉर्म DOCUMENT के साथ upload करना होगा
♦ Step #4
Now fill 91 more mobile number, email id and Aadhar card number in “Country code”. Column number 11 has to be left blank
अब “Country code” में 91 और mobile number , email id और आधार कार्ड नंबर fill करे कॉलम नंबर 11 को खाली रखना है
♦ Step #5
I/We fill in the name of the applicant in the number And fill the place date written Him self and sign.
I/We इमे आवेदक का नाम fill करे और Him self लिखे place date fill करे और signature करे
♦ Step #6
Here there is a box in front of every column, you have to right tick on the box which you want to change. As I want to change dob and photo signature, then I have ticked the box of dob and photo signature change only.
यहाँ पर हर कॉलम के आगे बॉक्स बना हुआ है आपको उसे बॉक्स पर right टिक करना है जो आप change करना चाहते हो जैसे में dob और photo signature change करना चाहता हु तो मेने only dob और photo signature change के बॉक्स को ही टिक किया है
