Sabse pehle aapako annexure-b form ka printout nikal lena hai printout aap ees link par click kar nikal sakte he annexure-b
- ab aapako form par applicant ka photo lagana he, applicant ke vohi photo lagaye jo photo aapane pan form me lagaya he
- photo ke upar company ke owner ya manager ka signature or seal lagaye
- “Sh./Smt/Kum”…….. me applicant ka name fill kre
- “son/daughter of”….. me applicant ke father ka name fill kre
- “Office Address”….. option me aapake office ka address fill karna he
- “Residential Address”…… option me applicant ke aadhar card ya applicant ke document ka address hi fill karna he
- “Registration Number of the Company/Institution etc”…. ees option me aapako aapani company ka Registration Number ya Gstin number fill karna he
- “PAN of the Company/Institution”….. me aapani company / office ka pan number fill kre

Details of the Individual issuing the certificate ees option me aapaki company ke owner ya manager me jis ka bhi aap signature kra rhe he unka details fill karna he agar aap owner ka signature karte he to aapako owner ka name or details fill karna he
- “Full Name” option me signature karne vale(owner ya manager) ka full name fill hoga
- “Designation” ees option me signature karne vale ka Designation(पद का नाम) fill kre
- “PAN of the Individual” ees me signature karne vale ka pan number fill karna he
- “Office address with location” option me office ka address fill karna he , mobile number , Date , Place fill kre
- “Signature” yaha par owner ya manager ka signature or seal (stamp , मोहर ) lagaye
